The Original Question:

Can you add a text block to a Squarespace Form Block?

Answer:

You cannot insert a Text Block into the Form Block. However, you can use Custom CSS to "hijack" the Description element of a Text Field to create the illusion of text box.

In the form here, I have a "Text Field" between the "Email Address" and "Message" fields. Using Custom CSS I've hidden the Text Field Title and Input, and formatted the description to be more distinct. Here's the sample CSS I've used:

#text-yui_3_17_2_3_1496093706376_4119 .title, #text-yui_3_17_2_3_1496093706376_4119 input {
  display: none;
}
#text-yui_3_17_2_3_1496093706376_4119 .description {
  font-size: 16px;
  opacity: 1;
}

This is the "fake" text box using the Description of a Text Field and customizing it it with Custom CSS. You can use the "Inspect" tool in your web browser to determine the ID of your particular Text Field, and then hide the Title and Input elements, and format the Description elemenet to your own tastes. It's important that you make sure this field is NOT marked as 'Required'!
